Overview
This project unites primary care and public health practice-based research networks (PBRNs) within four states to examine variation in the degree of primary care and public health integration across local jurisdictions, identify factors that may contribute to or impede integration, and assess whether areas of increased integration have better health outcomes. Primary Care PBRNs and Public Health PBRNs from Minnesota, Wisconsin, Colorado, and Washington participate in the study.
