2008 Survey

The drawing was held on August 8, 2008 and the winner will be notified via email.

Many thanks for taking the time out to attend the First Annual Public Health Systems & Services Research Keeneland Conference.  Your feedback is significant and will be taken into consideration as our team begins designing the Agenda and selecting the moderators, speakers and topics for next year. Please take a few moments to complete the survey below.  We appreciate your time and look forward to seeing you in 2009!

We presented the winner of our drawing with a copy of James Archembeault's  coffee table book, HISTORIC KENTUCKY.  Mr. Archambeault is "one of the most published photographers of Kentucky's beauty and the majesty of the horse."   We believe  this book will bring back fond memories of your trip to the Bluegrass State.

The survey will continue to be active until the end of 2008.  We may decide to hold another drawing later in the year.  Please check back for updates.
